Game Report

Semi Finals & Finals
Teams must use the NJYS State Cup Game Report.
Teams that fail to use the proper Game Report will be fined.Games played at the Home Team's Field
Some of the things you will need on game day are:
The scheduled game time is 1:00 PM unless multiple games are scheduled. If two or three games are scheduled on the same field, the first game shall be 12 noon. If mutually acceptable to both teams, games may be played earlier. There will be no night games without the prior approval of the Games Commissioner.
- Your portion of the referee fee
The game report. You can download a pdf copy or Word copy. You must use the official game report. Your team should fill in the team name and the game roster, i.e., Player Name, Player Pass Number, and Uniform Number. The team manager or coach should sign the report. The referee will complete the rest of the report and send the completed report to NJYS.
- Medical Releases for all your players
- Player and Coaches passes
- Proper behavior on the side lines
The winning team must report the game score to NJYS on the day the game was played. Click here to report the score online.
Any changes to the published scheduled dates must be approved ahead of time by the NJYS Games Commissioner.
Important Information for Teams playing Games at a Centralized LocationNJYS will schedule all quarter-final, semi-final and final matches. Quarter Final matches for the State Cup competition will be played at the designated home team's field. Quarter final matches for the Naitonal Championship competition may be scheduled by NJYS at a central location.
Items each team must have at these games are:
Your portion of the referee fee. NJYS will assign all referees to these matches and there will be a three referee crew for each match. At Quarter-Final matches, the teams are each responsible for one half of the referee fee as they were at all preliminary games. NJYS will pay referees for the semi-final and final matches.

Teams that are waiting for their match cannot warm up or congregate along the goal line of an active field or between fields. NJYS will designate an area for teams waiting for their match and teams are expected to stay within the area for their warm up activities. Teams not adhering to this requirement will be subject to disciplinary action and may be fined $100.00.
Check-In Procedures
Please review the required check-in activities for each round. Note: Players are only required to check-in at the Finals and not either Quarter-Finals or Semi-Finals.

* Blue Roster forms can be downloaded from the Region I web site. Instructions for completing the Blue Roster can also be found on the Region I
website. NJYS will provide blank sheets of blue paperAt any centralized location, referees will be assigned by the NJYS Referee Assignor.
A maximum of four (4) carded, rostered coaches, assistant coaches, administrators, trainers, etc for each team will be permitted on the sidelines with the rostered players. Sideline passes must be displayed prominently.
Spectator areas are behind the marked only area. Please be considerate of other games in progress. There is plenty of area for player warm up and preparations without congregating along the goal lines or between fields. Proper behavior is required on the sidelines.
