The Team Roster is the document that lists the players that are eligible to play for the team.  Until a player is officially rostered by a District Commissioner, he or she cannot play for that team.   Remember: You cannot have a team’s roster validated unless there is at least one coach listed on the roster that has a valid coach’s (volunteer or non-volunteer) pass.

Instructions on Completing the Roster

It is necessary to use a black ball point pen to give enough impression to go through the four layers of this form and black ink photocopies better. Typing is acceptable and welcomed.  Complete the roster by entering the following:

  1. League number. Click here for a listing of the travel leagues.

  2. Club number. Your club number can be found on your Club Affiliation Form.  The club number is a four-digit number between 9001 and 9800. If you do not have a club number, your club has not affiliated with NJYS.  Contact the NJYS Office to complete your affiliation and obtain a club number.

  3. League name. Click here for a listing of the travel leagues.

  4. Club Name (Full name, do not use initials)

  5. Team name

  6. Age Group (example: U12)

  7. Check box for Boys or Girls team - Girls teams can only have girls on the roster, any other team is a boys' team

  8. For each Coach, enter
    • Pass number.  The pass number has the form of NJ105031-C01.  The team portion of the pass number may be different if this is the second roster to which a coach is being rostered.
    • Coach’s Phone number
    • Name, Address, City, State, ZIP

  9. For each player, enter
    • Pass number (Just the position on roster – example 01 should be on first line).  For out of state players playing on this team, enter the pass number of the player's out of state pass.  If the pass does not have a number, leave the pass number blank.
    • Gender (M/F) • Date of birth (MM/DD/YYYY)
    • Name, Address, City, State, ZIP. 

Helpful Hints - Have the player sign the member pass before you enter the name on the roster form. The pass number must agree with the roster position number. Example: Pass Number NJ105031-12 is to be entered on line position #12.

* List all player pass numbers on the form in numerical order. For full-sided team, list up to 18 numbers. For U17-U19 teams, list up to 22 numbers and for small-sided teams, list up to 14 numbers.

* Using the Member Pass, NJYS Membership Form, and proof of age, fill in the name (last name, first name), birth date, and address for each player. Names should be listed alphabetically by last name.

Things you need to make sure of when you complete your roster

* You must initially roster a minimum of 11 players when you ask a District Commissioner to validate a full-sided or 22 player team roster. For a small-sided team roster, you must initially roster a minimum of 8 players.

* U8-U10 teams can have a maximum of 14 rostered players on its active roster at one time with a total of 17 players during the seasonal year, September 1 to August 31.

* U11-U16 teams can have a maximum of 18 rostered players on its active roster at one time with a total of 21 players during the seasonal year, September 1 to August 31.

* U17-U19 teams can have a maximum of 22 rostered players on its active roster at one time with a total of 25 players during the seasonal year, September 1 to August 31.

* Only U17, U18, U19 teams can have 22 player rosters.  Under no circumstances can a U16 team or younger have a 22 player roster.

After initial rostering of your team, additions or transfers to the team of any player or coach must be done with a NJYS Add/Transfer form.  Releases from the team must be done with a NJYS Release form.  Both of these forms are available on the NJYS website.  Instructions for completing these forms are on the form itself.  Attach a copy of all approved addition/transfer and release forms to your original pink roster.  Do not add names on pink rosters.

Once a player has been rostered to a team

    (1) he or she is a member of the team regardless if they play a game or not.
    (2) For teams that will playing in the National Championship Series or State Cup, a player has used their cup eligibility for the year.
