Membership Forms
Club Affiliation Form       Updated for 2007-2008 seasonal year,    previous year forms should not be used	Club Affiliation Form       Updated for 2007-2008 seasonal year,    previous year forms should not be used

The Non-Volunteer Membership Form must be completed through the NJYS Office.  District Commissioners cannot issue a Coaches pass for Non-Volunteers
