Player Membership Form

The player membership form is available online at the New Jersey Youth Soccer web site.  The following sections should be completed for each player before the roster is brought to the District Commissioner for validation:

1. First name and last name

2. Address  [City, State, Zip]

3. Phone Number

4. Date of Birth

5. Gender  [Put an 'X' next to appropriate gender]

6. League Name  [See League List]

7. League Number  [See League List]

8. Club Name 

9. Club Number

10. Team Number

11. Player Pass Number

12. Age Group, e.g., U12

13. Box titled Important. Parent's permission to participate-indemnification agreement. The player’s parent and the player must sign and date the form. The form must be signed before the player can be registered. Players who are eighteen or older may sign their own form. The District Commissioner will keep this form.

Volunteer Coach Membership Form

The volunteer coach membership form is available online at the New Jersey Youth Soccer web site.  The following sections must be completed by each volunteer coach before the roster is brought to the District Commissioner for validation:

1. First name and last name

2. Address

3. Phone Number

4. Email Address

5. League Name

6. League Number

7. Club Name

8. Club Number

9. Team Number

10. Coach Pass Number

11. Coach License Level

12. Box titled Important. Coach agreement. The coach must sign and date the form. The form must be signed before the coach can be registered.  The District Commissioner will keep this form.

Non-volunteer Coach Membership Form

This form can only be completed through the NJYS Office. District Commissioners cannot issue a Coaches pass to a Non-Volunteer. Packages for registering Non-Volunteers can be obtained from the NJYS office. The completed package must be returned to the office for processing which may require a week to complete. Non-volunteers cannot have their paperwork completed while they wait.
